Buying a subscription to MedConsents.com Medical Consent Forms is as easy as 1-2-3:

Click SIGN UP NOW. In order to select the correct pricing, you will need to enter the number of users who will use this product to obtain consent from a patient. This often is limited to the doctors and nurses who have direct patient care, but sometimes includes other staff. |
| For example: If your office has three doctors, and three nurses, and one pharmacist, who can consent patients, you have seven users total. You would purchase the subscription product below for 6-15 users. |
